Appointments. Please call 205-877-5444 between 8:00 a.m. and 5:00 p.m., Monday through Thursday, and Friday between 8:00 a.m. and 12:00 noon. It is best if you try to schedule your appointment in advance. If you have an urgent problem, we will see you as soon as possible.
Office Hours.
The office is open Monday through Thursday from 8:00 a.m. until 5:00 p.m., and on Friday from 8:00 a.m. until 12:00 noon. During these hours, registered nurses (R.N.) are readily available to handle your calls.
New Patients. Thank you for choosing Sparks & Favor, P.C. In order to expedite the registration process, please download and complete the new patient forms. Bring the completed forms along with your insurance card to your appointment. We recommend that new patients arrive 30 minutes in advance of their scheduled time. Prescriptions. If you need to have a prescription refilled, please call during regular office hours and be sure to have the telephone number of your pharmacy available.
We participate in a number of health insurance plans. If you are a member of one of the plans listed below, as a courtesy, we will bill your insurance company. Please note that the appropriate co-payment is due at the time of service.

Blue Cross / Blue Shield of Alabama. If you are insured by Blue Cross/Blue Shield of Alabama, we will bill Blue Cross for you. Only charges or balances not covered by your insurance will be billed directly to you.
Other Commercial Insurance. Our office operates on a cash basis for health insurance policies other than those specifically mentioned. We will accept VISA, Mastercard or personal checks at the time of your visit. We will provide you with all of the information needed to file a claim with your insurance company.
